and used by all GPs. Outside this we
will always charge less than is the
recommended going rate!
In all cases we need your written
consent to release medical details.
Forms should normally be available
by close of surgery two weeks from
their receipt. Ask reception for the
current time scale when handing in
your form – this work is done
outside of normal surgery (NHS)
hours.
